The Staff of Dr. Gary Evans would like to thank you for selecting our office to care for your dental needs. Our goals are to provide each patient with the highest quality dental care in a gentle, efficient, and pleasant manner; and to encourage prevention of future dental problems. We will actively listen to our patients and constantly improve our skills to provide a level of care never before offered in our industry.

Office Hours

Our office hours are arranged to provide you with a variety of appointment options. The office is staffed according to the following schedule:

    • Monday: Closed
    • Tuesday: 8:00am - 5:00pm
    • Wednesday: 8:00am - 5:00pm
    • Thursday: 8:00am - 5:00pm
    • Friday: 8:00am - 3:00pm
  • Saturday: 8:00am - 12:00pm (One a month)
Please complete the health questionnaire and bring it with you at your first visit. Also, so we may assist you in filing any claims ( if you have dental insurance), please bring your dental insurance card or a completed and signed dental claim form.

Fees & Payments

We ask that you make your portion of the payment due at the time services are rendered. If you are unable to do so, please discuss your situation with the front office staff. We will make every effort to accommodate unusual circumstances that make your financial obligations difficult to fulfill. Since we do not finance balances in house, we work closely with a highly recommended finance company called CareCredit. We will be happy to help you take the steps necessary in order for you to take advantage of their services. In addition, our staff will gladly go over their terms and payment plans. We also accept Visa, Mastercard, checks (with valid ID), and cash.

Dental Insurance

Your insurance contract is made between you and your insurance company. Insurance policies often do not provide full payment of dental costs. Insurance companies have their own fee schedules on which their payment is based. Payment by your insurance carrier may not be in line with what is actually charged for the services provided by our office.

We will bill your insurance carrier directly and you will be responsible for co-payments, deductibles, and non-covered services at the time of your appointment. Insurance plans are changing rapidly so please check with your insurance company if you have questions about your particular plan and benefits.

Finance & Other Charges

A carrying charge of $10.00 may be added to any account over 90 days or that doesn't honor a payment agreement.

Cancellations & No Show Appointments

24 hours notice is required for all cancellations or you will be charged $38.00 per hour of time scheduled.
